All-Inclusive Membership
To be paid in advance and partially refundable on a prorated basis. Covers unlimited visits during regular office hours (maximum of 30 minutes per appointment a day), four in home visits (must live within 20 minutes of the office), and a comprehensive annual exam. Routine labs are available at a low cost. It also provides direct access to the doctor by cell phone or email 24/7. After hours visits would be covered as well when needed and feasible. Virtual visits are also included at no extra charge. Every effort will be made to accommodate these members with same day or next day appointments when needed or requested.

Standard Membership
The membership includes a comprehensive annual exam for adults and well checks for children. Routine labs are available when needed at a low cost. Each additional office visit will be charged at a rate of $2/minute with a minimum visit time of 10 minutes. Appointments will be time based with the amount of time for the appointment chosen by the patient at the time of scheduling. The time of these appointments will be strictly enforced so that we can avoid excessive wait times for other patients that follow.

Out-of-Town Membership
This option is only available to our patients that are more than 60 miles from the office. You must already be an existing patient or have an initial consultation with Dr. Brooks at $8/min with a minimum of 30 min. All following consultations are $4/min. This is applicable to phone, virtual, and in office visits.
