Out of Town Membership
This option is available to our patients who live more than 60 miles from our office. You must already be an existing patient or have an initial consultation with Dr. Brooks at $8 per minute with a minimum of 30 minutes. All following consultations are $4 per minute. This is applicable to phone, virtual, and in office visits.
Out of Town Membership
$250/year for an Individual
$500/year for a Family (max 4 persons)
— $50/year for additional members to the Family membership (does not qualify toward individual plan)
