Standard Membership
The membership includes a comprehensive annual exam for men and women as indicated, and includes an EKG for patients aged fifty and above. Comprehensive labs and pap tests for women will be billed to insurance. If you do not have insurance, we will provide them to you at a low cost. All routine well child visits each year for children are included in this membership level. Vaccines are not included at this time. Each additional office visit will be charged at a rate of $2/minute with a minimum visit time of 10 minutes. Appointments will be time based with the amount of time for the appointment chosen by the patient at the time of scheduling. The time of these appointments will be strictly enforced so that we can avoid excessive wait times for other patients that follow.
