— Please note that filling out the enrollment forms does not mean you are a patient at Brooks Family Care. You must obtain approval before filling them out in order to be guaranteed as a patient.
— We have had a huge influx of new patient inquiries. We are so thankful for your interest in our practice, but it is taking us a little longer to get calls returned, so please be patient with us. You are welcome to email us as well, for a quicker response time.
